I quote this from FRCS 3E pg 256 under "Moving from One Plane to Another"

"All of these planes connect to Toril on the Material Plane in some way, whether through the coexistant overlap of the Astral, Ethereal, and Shadow planes or via portals through the AStral that connect to the deities', elemental, or energy planes. In a sense, the cosmology of Toril resembles a tree, with the branches (planes) growing from the center and some grranches having secondary branches (realms) growing from them. It is far easier and safer to take advantage of the existing connectivity to travel between planes, using the common ground of Toril as a stopping point, rather than foring a direct connection between two locations ...

"However, planar beings sometimes choose to force a direct path between two planes ... rather than use the existing paths. ... In addition, deities who are friendly to each other but live in different planes sometimes create permanent connections (portals) to their allies for convenience. Such artificial methods are generally beyond the reach of mortals."

Does this mean that spells like Plane Shift and Gate don't work, and that mortals with the Create Portal feat can't use it to link to another plane? If this is the case, then why does the very same tome say, "Creatures who touch or pass through the area of the portal are instantly teleported to the locale the portal's builder has specified. (The teleportation effect is similar to Teleport Without Error cast by a 17th-level caster, except that interplanar travel is possible.)"

Perhaps this means that the only permanent means for a mortal to build a link to another plane is a portal, but that other paths can be created by mortals between planes, but they are not permanent.

Gate and Plane Shift work normally with respect to the FR cosmology.

The channeled Astral plane really only poses a minor constraint solely with respect to Astral travel.

Thank you for your help, Kuje. Perhaps a Gate spell would only work on Toril or the Astral plane?

The Player's Guide expanded on this.

Basically you can planeshift through Faeruns, Mazticas, Kara-Turs, or Zakharas astral to the planes but if you are using one of those astrals you have to go to the plane itself, you can not change course to another plane in the middle of the journey. I.E. if you went from Faerun to Arvandor you could not then decide to go to the Abyss. You would have to return to Faerun and start all over.

Note: this isn't true for gates/portals.

I'm not sure about the gate spell though.
