Is there any magical way to transport someone from one world to another(I.e. Send someone from the realms to Greyhawk)?

There are gates between different worlds, or at least there were, the new cosmology is not my speciality. I asked a question yesterday about the city of Sschindylryn under Cormyr and part of the answer was this:
